Could large amounts of this popular sweetener affect the way your blood clots?

It’s in sugar-free gum, candy, baked goods and products marketed as keto-friendly or low-carb. Xylitol has also spent years enjoying a reputation as one of the more respectable alternatives to sugar.

But research is raising an uncomfortable question: Could large amounts of this popular sweetener affect the way your blood clots?

The answer isn’t settled, but the findings are significant enough that researchers say xylitol’s cardiovascular safety deserves a closer look.

Researchers found a connection to heart attack and stroke

In a study published in the European Heart Journal, researchers examined blood samples from more than 3,000 people undergoing cardiac evaluations.

People with higher circulating levels of xylitol were more likely to experience a major cardiovascular event such as a heart attack or stroke during the following three years. After accounting for traditional cardiovascular risk factors, those with the highest xylitol levels had a significantly higher risk of major cardiovascular events than those with the lowest levels.

But there’s an important distinction.

The finding doesn’t prove that eating xylitol caused those events. The human body naturally produces small amounts of xylitol, and people in the observational portion of the study weren’t being tracked according to how much xylitol they ate.

That’s why researchers didn’t stop there.

Xylitol appeared to make platelets more reactive

The team conducted laboratory experiments examining how xylitol affected platelets—the tiny blood components that help form clots.

Exposure to xylitol increased platelet responsiveness and appeared to make clotting easier under certain experimental conditions.

Researchers also gave 10 healthy volunteers a xylitol-sweetened drink. Their blood xylitol levels rose dramatically afterward, as expected. But researchers also detected increased measures of platelet responsiveness.

Those findings offer a possible biological explanation for the cardiovascular association, but they still don’t demonstrate that routinely consuming xylitol causes heart attacks or strokes.

Long-term clinical studies would be needed to answer that question.

Xylitol isn’t exactly an artificial sweetener

Xylitol is a sugar alcohol, or polyol.

Small amounts occur naturally in fruits and vegetables, and our bodies produce tiny quantities during normal metabolism. But the amount added to processed foods can be dramatically higher than anything naturally occurring in food.

Unlike intensely sweet sugar substitutes that require only tiny amounts, xylitol is similar in sweetness to table sugar. Manufacturers can therefore use substantial quantities to replace sugar while maintaining sweetness and texture.

It can appear in sugar-free gum, mints, candy, baked goods and other reduced-sugar products.

Because xylitol doesn’t raise blood glucose the same way ordinary sugar does, it has also become popular in some low-carb and diabetes-oriented foods.

This doesn’t mean you need to panic about your chewing gum

It’s easy to take a finding like this and immediately start emptying the pantry. The research doesn’t support that reaction.

Scientists haven’t established how much dietary xylitol, if any, would meaningfully increase cardiovascular risk over time.

The study’s observational participants were already undergoing cardiac evaluations, meaning they weren’t necessarily representative of the general population. The small intervention study involved only 10 healthy volunteers and examined short-term changes in platelet activity—not heart attacks or strokes.

Researchers themselves concluded that further studies are needed to evaluate xylitol’s long-term cardiovascular safety.

The bigger question may be how much we’re consuming

Xylitol isn’t inherently foreign to the human body.

The question is whether consuming concentrated quantities as a sugar substitute produces biological effects that are different from encountering tiny amounts naturally. That’s becoming increasingly relevant as consumers seek products labeled “sugar-free,” “keto” and “low carb.”

Replacing sugar doesn’t automatically make a food metabolically neutral.

Sugar substitutes can have their own effects in the body, and researchers are still learning what some of those effects may be.

There’s another xylitol warning dog owners should know

Whatever eventually happens with the human cardiovascular research, one xylitol warning is already firmly established.

Xylitol is extremely dangerous to dogs.

Even relatively small amounts can trigger a rapid release of insulin, causing severe hypoglycemia, and larger exposures can lead to liver failure.

Sugar-free gum and candy containing xylitol should therefore be kept well out of a dog’s reach.

Final word

Xylitol isn’t suddenly “poison,” and this study doesn’t prove that chewing sugar-free gum will give you a heart attack.

But it does challenge an assumption many of us make about food: If something contains less sugar, it must automatically be healthier.

Nutrition rarely works that neatly.

For people consuming xylitol occasionally, the research isn’t a reason to panic. But if you’re routinely eating large amounts of foods sweetened with sugar alcohols, checking the ingredient list may be worthwhile.

The most interesting lesson may extend beyond xylitol itself.

When manufacturers remove something from our food, what they put in its place matters too.
