What if clues to Alzheimer’s disease could be found while someone was still in their 50s or 60s—and feeling perfectly fine? Researchers are getting closer to answering that question.
A new study found that Alzheimer’s-related proteins in the blood were already associated with differences in memory and thinking during midlife, potentially years before dementia would normally be diagnosed.
Researchers studied people in their 50s and 60s
The NIH-funded research involved 1,350 participants from the long-running CARDIA study. None had been diagnosed with Alzheimer’s disease.
Researchers measured several forms of amyloid-beta and tau—proteins associated with the characteristic plaques and tangles found in Alzheimer’s disease.
According to the National Institutes of Health, between 4% and 15% of participants had biomarker levels high enough to suggest Alzheimer’s-related changes in the brain.
People with higher levels of certain Alzheimer’s-related blood biomarkers tended to perform worse on tests of memory and thinking.
Researchers also had earlier cognitive testing for many participants, allowing them to examine changes over approximately five years.
Alzheimer’s can begin long before memory problems
One of the biggest challenges in treating Alzheimer’s is timing. By the time significant memory loss appears, biological changes may have been developing inside the brain for many years.
Amyloid plaques and tau tangles can accumulate long before dementia becomes obvious.
That means the ideal time to intervene could potentially be years before someone realizes anything is wrong.
Historically, detecting those changes required expensive brain scans or spinal-fluid testing. Blood biomarkers could eventually offer a much simpler option.
Blood tests for Alzheimer’s are advancing quickly
This isn’t the only recent breakthrough. Another NIH-supported project developed an Alzheimer’s “clock” using a blood biomarker called p-tau217.
As the NIH reported earlier this year, researchers studying older adults found that changes in p-tau217 could help estimate when Alzheimer’s symptoms might emerge.
The prediction wasn’t precise enough to circle a birthday on the calendar, but the model estimated symptom onset within roughly three to four years on average.
Together, studies like these are moving Alzheimer’s diagnosis increasingly toward blood-based testing.

Earlier detection matters more now
For decades, diagnosing Alzheimer’s earlier offered limited practical benefit because there were few treatments capable of changing the disease process. That’s beginning to change.
Newer Alzheimer’s drugs can slow disease progression in some patients with early disease, making accurate and timely diagnosis increasingly important. Researchers are also studying treatments aimed at people before significant symptoms appear.
If those approaches prove successful, a simple blood test could eventually become an extraordinarily important tool for identifying people early enough to benefit.
We’re not there yet. But the pieces are beginning to come together.
Your everyday brain health still matters
No blood test replaces the basics of protecting cardiovascular and cognitive health. Physical activity, blood-pressure management, good sleep, social engagement, avoiding smoking and managing diabetes and cholesterol all remain important.
No lifestyle guarantees that someone won’t develop Alzheimer’s. But many of the factors that support heart health also appear to support brain health.

The remarkable thing about this research isn’t simply that scientists found Alzheimer’s-related proteins in blood.
It’s how early those proteins may begin telling a story.
For generations, Alzheimer’s has been diagnosed largely by what happens after memory begins to fail. The future may look very different.
Doctors may someday identify biological changes years earlier, while people are still working, traveling, raising families and living independently. That possibility brings difficult questions about treatment, prediction and whether everyone wants to know.
But it also brings something Alzheimer’s medicine has desperately needed: Time.
